The Lakeview Family Health Team (LVFHT) welcomes and encourages people with disabilities to use its services and will make all reasonable efforts to meet the needs of people with disabilities.  Access to programs and services for people with disabilities will be provided in a manner that respects the rights to dignity, independence and intergration.  The policy is intended to ensure that the Lakeview Family Health Team (LVFHT) meets or exceeds the standards set by the Accessibility for Ontarians with Disabilities Act, 2005 ("AODA") and the Integrated Standards Regulations (IASR) contained therein.  Persons with disabilities will be given an opportunity equal to that given to others to obtain, use and benefit from the programs and services.

Welcome and encourge individuals with disabilities to our team and will make all reasonable efforts to meet the needs of people with disabilities.  Access to programs and services for people with disabilities will be provided in a way that respects their rights to dignity, independence, and integration.

The definition of disability in the

Accessibility for Ontarians with Disabilities Act, 2005 ("AODA") can include:

• Physical disability, infirmity, malformation or disfigurement,
• Mental impairment, disorder or developmental disability,
• Learning disability,
• Injury or disability for which government benefits are received

The Lakeview Family Health Team (LVFHT)

will strive to provide service to everyone in a welcoming and supportive environment.

The unique needs of individuals with disabilities will be considered by:

• Applying the LVFHT policies and procedures to specific circumstances
• Communicating with people in a way that meets their needs
• Incorporating feedback and suggestions in a variety of formats
• Welcoming accompanying support people and service animals
• Offering a range of assistive devices such as wheelchairs
• Informing the community if services are not available where possible,
• Ensuring that emergency responses address accessibility
• Training staff and volunteers to provide the best possible service
• Providing a forum to address concerns or challenges associated with accessing our services
• Facilitating opportunities for continuous improvement
