The Lakeview Family Health Team is offering a unique opportunity for a Primary Care Physician to join an interdisciplinary, collaborative health care team in southeastern Ontario. Positions are available at all three locations including Brighton, Colborne, and Quinte West (Trenton).
The FHT is committed to looking after their patients and use a collaborative model of care with a team of Allied Health professionals. We have a Nurse Practitioner, Nurses, a Diabetic Educator, a Physician Assistant, a Social/Mental Health Worker, and Dietitian, supported by administrative staff at all sites.
The FHT is governed by a Board consisting of the physicians from all sites, an Allied Health Professional, and two community members.
The FHT is directly funded by the Ministry of Health and Long Term Care. The physicians’ compensation is based on a Family Health Organization model (FHO). There is a Lead physician who administers this.
