Having a Quality Improvement Committee, a Quality and Patient Safety Policy,and a Patient Complaint Policy in place, as required by Ontario Health

Submitting a Quality Improvement Plan to Ontario Health annually

Monitoring patient satisfaction and patient health outcomes quarterly, then reporting the results on Scorecards that are reviewed by the Quality Improvement Committee and the Board of Directors

Designing our programs and services to enhance accessibility by ensuring that health information handouts, signage, and surveys are easy to read, and by providing information in alternative formats

Improving access by offering some services to patients who are not part of our team, such as online group mental health and wellness sessions, and mass vaccination clinics

Using inclusive language and ensuring that patients from equity-seeking groups are welcomed and treated with respect

Supporting provider and staff satisfaction and wellbeing by holding regular team meetings, team retreats, and training sessions

Leading the region in using new technologies that help patients while improving efficiency and reducing costs, such as online booking, virtual visits, and e-referrals

Ensuring that the Patient Rights and Responsibilities Policy and the Patient, Family and Caregiver Declaration of Values are communicated to all patients, caregivers, and team members
